COVID-19 Vaccine Requirement
All UW students, including non-degree students, are required to be vaccinated against COVID-19 unless they receive a medical or religious exemption. You must provide proof of vaccination or submit a request for an exemption before you can be enrolled from “standby status.”
Vaccinated Students
Vaccinated students must verify their vaccination status through a secure student attestation form (UW NetID required). After answering a few questions about your vaccination, you’ll be asked to upload proof of vaccination, such as a vaccination card or other vaccination record.
Students Requesting an Exemption
In addition to the student attestation form, students who are requesting an exemption for medical reasons or deeply held religious beliefs must complete a COVID-19 Medical or Religious Exemption Request. Philosophical exemptions are no longer allowed by Washington state.
Deadlines
If you enrolled at the UW in autumn quarter, you had to verify your vaccination status or request an exemption by November 5, 2021, or a hold was placed on your account for winter quarter registration.
If you’re enrolling at the UW for the first time in winter quarter, you must provide proof of vaccination or submit a request for an exemption before you can register for classes.
